Since 2012, I have been working as an Operating Theatre Assistant in Australia. Our days as Operating Theatre Assistants are filled with constant unpredictable emergencies, all with unknown outcomes—most good and some not so good. The demands are constant, the decisions can be critical, and the expectation to perform at the highest level never disappears. Yet they continue to learn, adapt and strive for better outcomes for every patient who enters the operating theatre. They save lives, restore health and support patients through some of their most vulnerable moments..

How we fit in
As an Operating Theatre Assistant, we provide essential services that enable our incredible clinical teams to transition seamlessly from one critical surgical procedure to the next random emergency, effectively eliminating surgical stress. Our responsibilities include setting up operating theatres, positioning patients, assisting the surgical team, scrubbing out wounds, applying traction to breaks, holding for prep, and much more.
We work alongside surgeons, anaesthetic professionals, nurses, technicians and other clinical staff with one shared purpose: to ensure the patient and surgical team have the environment, equipment and support required to perform safely and effectively. We prepare. We anticipate. We adapt. We solve problems. We keep surgery moving.

Experience in the operating theatre is more than knowing where equipment is stored or how a theatre is set up. For an Operating Theatre Assistant (OTA), experience becomes a practical form of knowledge—the ability to recognise what is happening, anticipate what may happen next and act effectively when circumstances change.

Experience in the operating theatre is more than knowing where equipment is stored or how a theatre is set up. For an Operating Theatre Assistant (OTA), experience becomes a practical form of knowledge—the ability to recognise what is happening, anticipate what may happen next and act effectively when circumstances change.
The perioperative environment is dynamic. Surgical lists change, emergencies arrive, equipment fails, positioning requirements become more complex and an apparently routine procedure can quickly demand additional resources. An experienced OTA learns to read these situations through observation, repetition and involvement.
Surgical readiness begins before the patient enters the theatre. The OTA considers the procedure, operating table configuration, positioning equipment, patient-transfer requirements, specialist equipment and potential complications. Rather than simply responding to requests, experience encourages anticipation: What will this surgical team need next? What could delay this procedure? What can I prepare now?
Under pressure, this experience becomes particularly valuable. Effective OTAs develop situational awareness while remaining calm, communicating clearly and prioritising patient and surgical-team requirements. They learn when immediate action is required, when clarification is necessary and when a developing problem should be escalated to the appropriate clinical professional.
Importantly, experience does not replace clinical governance, established procedures, manufacturer instructions or professional scope. Instead, experience strengthens the OTA's ability to work effectively within them.
Every theatre setup, patient position, emergency response, equipment problem and interaction with experienced perioperative professionals contributes to a growing knowledge base.
Over time, the OTA begins to see beyond individual tasks and understand the flow of surgery itself.
That is experience-driven perioperative knowledge: observe, prepare, anticipate, communicate and respond.
The goal is simple but demanding—help create an operating theatre that is surgically ready before the need becomes urgent.
